I should note that this scene was probably a tribute to a similar scene in Yu Yu Hakusho. With Bleach, Ichigo and Orihime got married, and although many fans wanted to see him end up with Rukia, the marriage we did get makes a lot more sense.

Ichigo may have gone to Soul Society to save her from execution and put his life on the line, but that was out of strong friendship and gratitude that Rukia gave him the means to protect his family. Renji almost lost his life and put everything on the line for Rukia. The earliest moment for Rukia was when she announced her potential adoption to Renji. She was crushed upon thinking he was happy, because she expected a different reaction as she wanted the separation to sadden him and make him fight for her.
But he only reacted in the way he believed was best for her, and that ended up making him feel he put her on the path towards death. If she just wanted friendship, why did she expect him to protest? Ichigo and Rukia allowed Orihime and Sora to make their peace before he was purified and sent to the afterlife. Sometime later, a swarm of Hollows invaded their town, leaving Ichigo in a difficult place as all of his friends and loved ones were at risk of being killed by them.
When Orihime and Tatsuki were attacked by a Hollow, Orihime revealed her own latent spiritual powers and defeated it. She then decides to join Ichigo on his adventures.
When the Arrancar unmasked Hollows invade, Orihime is badly beaten, leaving Ichigo feeling guilty about not protecting her. Both resolve to improve their skills in preparation for the next assault, and in the meantime Rukia got Ichigo to specifically protect Orihime as compensation for not having been able to be there for her.
